To seek assistance from Uptown People's Law Center, please complete the pre-screen form below. We use pre-screen forms to gather basic information about the concerns that you are having and to determine whether you are eligible for services. It is not a guarantee that services will be provided. After you complete the pre-screen form our advocacy & intake coordinator will pass your information along to our paralegals and attorneys who will decide the best course of action.
You can:

- Download the form and send it to us by email at uplc@uplcchicago.org, or bring it to our office on walk-in days (Tuesdays and Thursdays from 10 AM to 4 PM).

Para buscar ayuda de Uptown People’s Law Center, favor de completar el formulario de pre-selección abajo.
